insurance and sliding scale

I am an out-of-network provider and do not work with any insurance companies. If you have the option of using out-of-network benefits, I do find that most insurance companies will reimburse for individual therapy services, and I will provide monthly statements with all the coding and relevant details insurance companies require. I also understand the importance of accessibility and flexibility in mental health, and I can accommodate clients with a range of financial situations by offering a sliding scale fee structure. Please don’t hesitate to inquire.

Why I don’t take insurance for couples therapy

Short answer: insurance is built for treating one person’s diagnosis, not for treating a relationship. Most insurers require:

  • an identified patient,

  • a mental-health diagnosis for that person, and

  • proof the treatment is medically necessary and aimed at that diagnosis.

While that model fits individual therapy, I don’t believe it fits couples therapy, where we’re focused on patterns, communication, and connection (not treating one partner’s disorder!). I believe that in couples work, the relationship is the client. And while there is a code for “relationship problems” (Z63.0), insurers generally don’t cover it.

Could we bill under one partner’s diagnosis? I choose not to. Doing so would mean labeling one partner the “patient” and documenting treatment as if the work primarily targets their condition. Some therapists make different calls, and I can respect that. For me, that route isn’t accurate to what we’re doing together, and I believe it can create ethical and privacy concerns.
